Hardware: Is the hardware Croda provides sufficient for you to adequately perform your job? Choose 1 to 5 1 No - I can barely survive with it 2 Just barely 3 Seems fine and my needs are moderate 4 Yes, I am content 5 Aboslutely, no complaints at all
Software: Are you provided with the necessary software to perform your job? Choose 1 to 5 1 No, my needs are completely un-met 2 Sort of, but would like some changes 3 Yes, I have all I need 4 Yes, and you have even provided programs for my special needs 5 Yes, I have all I need and all I ever request
Courteousness: Does the IT staff treat you with the courtesy and respect you deserve? Choose 1 to 5 1 No, I feel as though I am treated like dirt 2 Courteous - but to a fault 3 Yes, no complaints with this 4 Yes, I always feel well treated 5 Absolutely, the staff could not behave more professionally
Speed: How quickly does IT attempt to address your support needs? Choose 1 to 5 1 Far too long, it always seems to take forever to get anything 2 Too slow, I always need to repeat requests 3 Pretty quick considering staff levels 4 Quick - I never need to wait too long 5 Very fast - I am impressed with the speed
Quality: Speed of resolution aside, how effective is IT in resolving your support issues? Choose 1 to 5 1 Quite Bad, almost nothing is ever resolved 2 Not good, a mix of success and bothersome failures 3 Fine, time aside - most everything is usually resolved 4 Good, all of my issues are reasonably addressed 5 Very Good, Everything is always resolved
Overall: Provide an overall rating for how supportive IT is in helping you do your job effectively. Choose 1 to 5 1 Very Poor, they just don't care 2 OK, but they really should be more attentive to my needs 3 Fine, I can tell they care and want to help 4 Good, They are very supportive of my needs 5 Very Good, they treat me like an important customer
Overall 2: How does your Overall rating above compare with December of 2005? Choose 1 to 5 1 N/A - I did not work for Croda last year 2 Worse, things were noticably better then 3 About the same, I can't note a difference 4 Better, an overall service improvement 5 Much better, a significant improvement is noticed
